The proposed project for my organization that will improve patient outcomes is the electronic health records. For a long time, our organization has been using paperwork to document patient information and this has not only been a hindrance to fast service delivery but also a source of medical errors. Paperwork is a tedious method of keeping patient data because it takes long to retrieve and therefore, it is not effective during emergencies or when urgent data is required. During patient admission, it is necessary to identify specific information about a patient including previous treatments, adverse drug reaction as well as other underlying conditions that may help in designing a treatment plan, both of which are not possible with the use of paperwork. Also, the use paperwork may cause illegible handwritings that can cause a mis-prescription of drugs leading to medication errors. Paperwork is also vulnerable to damage by water therefore rendering it obsolete in its application in healthcare.
Another problem with the use of paperwork is the breach of patient privacy as there are no security features that can be implemented to prevent the unauthorized access of patient information. With all the challenges associated with paperwork, the introduction of electronic health records will help in improving patient outcomes (Kruse et al., 2018).

Several stakeholders will be affected by the implemented project in various aspects and therefore, it will be ideal to involve them in the implementation process. Some of the stakeholders to be affected by the project include nurse informaticists, patients, nurses and facility managers. Even though there are several other stakeholders to be impacted by the project,. The aforementioned stakeholder will be the most affected.
The nurse informaticists will be affected by the project in that with its implementation, they will be required to continuously gather information, analyze it and develop strategies and other healthcare features meant to improve patient outcomes. For instance, nurse informaticists can use the data to identify causes of hospital acquired pressure ulcers and develop appropriate interventions. Patients will also be affected by the project in that the implementation will help in facilitating the sharing of patient data among various departments which will lead to improved patient outcomes.
The nurses will be impacted by the project given that they will be the primary users of the EHR and hence, there will b a need for training to help them gain knowledge and skills to use the EHR system. The facility managers will be impacted in that they will be required t offer the financial input required to facilitate the implementation of the project.

The implementation of the electronic health records will enhance patient care efficiencies in various ways. First, there will be easy retrieval of patient information which will facilitate easy admission and discharge of patients. Electronic health records can harmonize patient information which facilitates easy retrieval. Besides, there will be easy sharing of patient information between the various departments therefore promoting a holistic patient centered care. Holistic care is critical because it helps in combining the inputs of various professionals to treat patients (Lye et al., 2018).
Also, electronic health records will help overcome medical errors that are associated with paperwork due to illegible handwritings. Poor handwritings can cause serious problems including prescription errors because various healthcare providers have varied handwriting some of which is illegible (Barbour, 2019). The electronic health records have also a patient portal that can facilitate effective communication between patients and healthcare providers therefore improving after discharge care.
